Abstract:Super minimally invasive surgery (SMIS) is a new stage in the history of medical surgery, as well as the ultimate concept of therapeutics in the future. The principle of SMIS is based on maintaining the anatomical structure of the human body to remove diseases.which means that the patient is restored to the original state after surgery, and the whole body is not significantly different before and after treatment. Under the instruction of SMIS, patients’ quality of life remains the same, and the survival time is not affected by the disease, completely different from the traditional surgery and minimally invasive surgery (MIS) which needs to remove organs at the cost of the operation mode. At present, there is no review or related studies on the different treatment channels of SMIS. In this review, we will review the update anvances in different treatment channels of SMIS by digestive endoscopy. The channels of digestive endoscopy SMIS can be divided into four categories, including single channel of digestive tract, double channel of multi-cavity, tunneling technique channel and aspiration biopsy channel. Based on these channels, new surgical methods of SMIS will be studied in the future. The new progress in recent years are as follows. Such as single channel of digestive tract, autologous skin-grafting surgery to prevent large area postoperative stricture after ESD, endoscopic retrograde appendicitis therapy (ERAT) used in the treatment of acute appendicitis, the duodenal papilla occlusion by SureClip from MicroTech after EST works as "papilla remolding", retaining the sphincter pressure and anti-reflux barrier function after EST, the treatment of intrahepatic bile duct and gallbladder calculi by endoscopic retrograde cholangiopancreatic duct, etc. In the field of multi-lacunae and double channels, gastroscope combined with thoracoscope and gastroscope combined with laparoscope are used in the resection of large benign tumors, etc. In the field of tunnel technique, diverticular peroral endoscopic myotomy (D-POEM) treats esophageal diverticulum, and gastric peroral endoscopic pyloromyotomy (G-POEM) treats gastroparesis, etc. In the field of aspiration biopsy channels, the aspiration channels of digestive organs have been used for temporary drainage and interventional therapy, and the application of percutaneous transhepatic cholangioscopic have been used in the diagnosis and treatment of biliary diseases. These surgeries extend the concept of SMIS applied in clinical treatment.
