    目的 研究接收时和清洗消毒后115条外来内镜钳子管道的微生物污染状况,初步了解其真实消毒质量,分析存在的问题并提出建议。方法 选取2022年6月-2023年12月期间武汉大学人民医院消化内镜中心的115条外来内镜作为研究对象,在接收时和清洗消毒后,分别对外来内镜的钳子管道进行微生物采样及菌种鉴定,比较两者的消毒合格率、微生物检出率和菌群情况。结果 外来内镜清洗消毒后的总体消毒合格率(91.3%)高于接收时(57.4%,P<0.001)。清洗消毒后总体微生物检出率低于接收时(13.0% VS 48.7%,P<0.001),清洗消毒后高致病微生物检出率和低致病微生物检出率均低于接收时(均P<0.001)。接收时和清洗消毒后检测出消毒不合格的外来内镜分别为49条、10条,经微生物菌种鉴定,接收时和清洗消毒后检出率居首位的是假单胞菌属。清洗消毒后假单胞菌属的数量(5株)低于接收时(24株,P<0.001),但其他菌群比较,差异均无统计学意义(均P>0.05)。结论 外来内镜在接收时消毒合格率低,微生物检出率高,存在较大交叉感染风险;清洗消毒后,消毒合格率提高,微生物检出率降低,大大降低了患者感染风险。因此,应重视外来内镜投入临床使用前的消毒质量监测,建议其接收时先严格按照WS507-2016《软式内镜清洗消毒技术规范》要求进行清洗消毒,然后对其进行生物学监测,监测合格方可使用。

    Abstract:

    OBJECTIVE To study the microbiological contamination of 115 endoscopic forceps channel at the time of reception and after disinfection, and gain a preliminary understanding of their real disinfection quality, then analyse the problems and make recommendations. METHODS 115 endoscopes received in the Digestive Endoscopy Center, Renmin Hospital of Wuhan University were chosen. Microbiological sampling and strain identification of the endoscopic forceps channel were carried out at the time of reception of the endoscopes received and after cleaning and disinfection, respectively, then the qualified disinfection rate, microbial detection rate and flora situation of endoscopes received were compared between the two. RESULTS The overall qualified disinfection rate after cleaning and disinfection of endoscopes received was higher (91.3%) than that at receiving (57.4%, P < 0.001). The overall microbiological detection was lower after cleaning and disinfection than that at reception (13.0% vs 48.7%, P <0.001), the detection rates of high pathogenic organisms and low pathogenic organisms were lower after cleaning and disinfection than that at reception (both P < 0.001). The number of endoscopes received with disinfection failures detected at receiving and completing cleaning and disinfection was 49 and 10, respectively, and Pseudomonas spp. were the most predominant species detected by microbial strain identification. The number of Pseudomonas spp. was lower after cleaning and disinfection (5 strains) than that at reception (24 strains, P < 0.001), but none of the differences were statistically significant when compared to the other flora (all P > 0.05). CONCLUSION At the time of receiving the endoscopes received, the qualified disinfection rate was low and the microbiological detection rate was high, and there may become a greater risk of cross-infection; after cleaning and disinfection, the qualified disinfection rate was increased and the microbiological detection rate was decreased,which may greatly reduce the risk of patient infection. Therefore, attention should be paid to the disinfection quality monitoring of endoscopes received before putting into clinical use, and it is recommended that they should first be cleaned and disinfected in strict accordance with the requirements of WS507-2016 "Technical Specification for Cleaning and Disinfection of Flexible Endoscopes" at receiving, and then biologically monitored.They can be used if they pass the monitoring.
